1. is very possible, because orbiting speed is about a 20-30 km/sec and any debris, bigger than sand drop, in that speed may penetrate astronaut and pass thru a space ship.
That is why working a programs like "Let free a space from human garbage" (garbage make a space travels WERY dangerous)

granted the debris s moving jolly quick, but they tend to have small mass. and im fairly sure that nasa have anticipated this problem, and taken appropriate preventative measures.
